มาตรา 1188
- หนังสือตั้งผู้รับฉันทะนั้น ให้ลงวันและลงลายมือชื่อผู้ถือหุ้น และให้มีรายการดังต่อไปนี้ คือ
- (๑) จำนวนหุ้นซึ่งผู้มอบฉันทะนั้นถืออยู่
- (๒) ชื่อผู้รับฉันทะ
- (๓) ตั้งผู้รับมอบฉันทะนั้นเพื่อการประชุมครั้งคราวใด ฤๅตั้งไว้ชั่วระยะเวลาเพียงใด

- The instrument appointing a proxy shall be dated and signed by the shareholder and shall contain the following particulars:
- (1) The number of shares held by the shareholder.
- (2) The name of the proxy
- (3) The meeting or meetings or the period for which the proxy is appointed.
